| Cost
The fee for an assessment is $1,500. When a family has insurance coverage
we will file for insurance payment. We recognize that, even with insurance,
families may not be able to afford the cost of the assessment. As a result
we have developed a payment scale based on income. This fee scale ranges
for $100 to $1,500 depending on ability to pay. A child’s eligibility
for assessment is not based on your insurance coverage.
Appointments
A child’s parent or guardian may call the intake coordinator at
214.915.4700 Monday through Friday in order to exchange information. The
intake coordinator will forward the information to ICCAP and you will
be contacted to schedule an appointment.
